GLP-1 for Men: How It Impacts Testosterone and Fat Loss

GLP-1 medications have changed the way doctors approach obesity and metabolic health. Drugs such as semaglutide, along with dual GIP/GLP-1 medications such as tirzepatide, can produce substantial weight loss in appropriate patients.
For men, however, losing weight is only part of the conversation.
Many men considering a GLP-1 medication also want to know what it could do to their testosterone, muscle mass, energy, libido, and long-term body composition. Some are already being evaluated for low testosterone, while others worry that rapid weight loss could cause their hormone levels to fall.
The relationship between GLP-1 medications and testosterone in men is more encouraging than many people expect. Current research suggests that men with obesity-related low testosterone may actually experience an improvement in testosterone levels as they lose excess body fat and improve their metabolic health. At the same time, significant weight loss can include some loss of lean mass, making strength training, nutrition, and medical monitoring important.
Do GLP-1 Medications Lower Testosterone in Men?
Current evidence does not suggest that GLP-1 medications routinely lower testosterone in men. In overweight or obese men—especially those with metabolic dysfunction—the opposite may occur.
A 2025 systematic review and meta-analysis evaluating GLP-1 receptor agonists found increases in total testosterone and several other reproductive hormone measurements among men receiving treatment. Researchers noted, however, that the available studies were relatively small and that it remains uncertain how much of the improvement comes directly from the medication versus the weight loss it produces.
Another 2025 meta-analysis found that GLP-1 therapy was associated with higher total and bioavailable testosterone, although results for free testosterone and sex hormone-binding globulin were less consistent. The authors emphasized that larger randomized trials are still needed.
More recent clinical data have pointed in the same direction. In a retrospective study of 215 men taking incretin-based weight-loss medications, average total testosterone increased from approximately 332 ng/dL to 399 ng/dL. Men who lost the most weight generally experienced the greatest improvement in testosterone.
That does not mean GLP-1 therapy should be considered a testosterone treatment. It does suggest that treating excess weight and metabolic dysfunction may improve one of the underlying causes of low testosterone in some men.
Why Obesity Can Reduce Testosterone
Excess body fat can influence testosterone through several overlapping mechanisms.
Obesity is commonly associated with insulin resistance, inflammation, sleep apnea, reduced physical activity, and disruption of the hypothalamic-pituitary-gonadal axis that regulates testosterone production. Greater adipose tissue can also increase conversion of testosterone into estrogen through aromatase activity.
As body fat decreases and metabolic health improves, some of these suppressive effects can improve as well.
This is why a man with obesity-related functional hypogonadism may see his testosterone rise after significant weight loss—even without starting testosterone replacement therapy.
The important distinction is that this is not true for every cause of low testosterone. Men with primary testicular failure, pituitary disorders, medication-related suppression, genetic conditions, or other forms of hypogonadism may still require separate evaluation and treatment.
GLP-1 Medications Can Produce Significant Fat Loss
GLP-1-based medications help reduce appetite, increase satiety, slow gastric emptying, and improve glucose regulation. For many patients, this makes maintaining a calorie deficit substantially easier than with lifestyle changes alone.
The result can be meaningful reductions in total body weight and visceral fat.
Body-composition data from the SURMOUNT-1 tirzepatide substudy are especially helpful for understanding what that weight loss consists of. Participants treated with tirzepatide experienced a 21.3% reduction in body weight over 72 weeks. Fat mass decreased by 33.9%, while lean mass decreased by 10.9%. Researchers estimated that approximately 75% of the weight lost was fat and about 25% was lean mass.
For men focused on improving body composition, the goal therefore should not simply be to make the number on the scale fall as quickly as possible. Ideally, treatment should maximize fat loss while preserving as much muscle as possible.
Will You Lose Muscle on a GLP-1?
Some lean-mass loss occurs during virtually any substantial weight-loss program. GLP-1 therapy is no exception.
The concern becomes greater when weight loss is rapid, calorie intake becomes excessively low, protein intake is inadequate, or resistance training is absent.
Men who want to preserve muscle while using a GLP-1 medication should continue progressive resistance training whenever medically appropriate and make adequate nutrition a priority. The goal should be sustainable fat loss rather than simply eating as little as possible.
This becomes particularly important for men over 40, men who are already relatively muscular, and men using testosterone replacement therapy who want to improve body composition without giving up hard-earned lean mass.
A medical weight-loss program should therefore look beyond total pounds lost. Waist circumference, strength, body composition, dietary intake, energy levels, and laboratory markers can all provide useful information about whether the weight-loss strategy is producing the desired result.
Can Raising Testosterone Make Fat Loss Easier?
Testosterone and body composition are closely related, but testosterone is not a substitute for a weight-loss program.
Men with true testosterone deficiency may experience changes including reduced lean mass, increased body fat, lower energy, decreased libido, and impaired exercise performance. Appropriately treating confirmed hypogonadism may help improve some of these factors.
At the same time, losing excess body fat may help normalize testosterone in men whose low levels are primarily related to obesity.
This creates a potentially beneficial cycle: improved metabolic health can support healthier testosterone levels, while maintaining healthy testosterone levels may help men preserve muscle, remain physically active, and improve overall body composition.
The key is determining which problem is actually present rather than assuming every overweight man with fatigue needs testosterone.
Can You Use a GLP-1 Medication and TRT Together?
GLP-1 medications and testosterone replacement therapy serve different purposes.
GLP-1-based medications are used for conditions such as obesity, overweight with qualifying health risks, and type 2 diabetes depending on the specific medication and indication. Testosterone replacement therapy is intended for appropriately diagnosed testosterone deficiency.
Some men may legitimately qualify for both treatments.
For example, a man with obesity and repeatedly confirmed low testosterone accompanied by appropriate symptoms may benefit from addressing both metabolic health and hypogonadism under medical supervision.
Other men may discover that substantial weight loss improves their natural testosterone enough that their hormonal status should be reassessed.
Men already taking TRT should not stop or change their testosterone simply because they begin losing weight. Hormone levels, symptoms, hematocrit, and other relevant laboratory measurements should be followed by the prescribing clinician so treatment can be adjusted when appropriate.
What Happens to Estrogen as Men Lose Body Fat?
Men naturally produce estrogen, much of it through conversion of testosterone by the aromatase enzyme.
Because adipose tissue contributes to aromatase activity, substantial fat loss may change the balance between testosterone and estrogen in some men. This is another reason hormone treatment should be based on symptoms and laboratory testing rather than fixed assumptions.
A man who loses 30 or 40 pounds may not have the same hormone requirements or metabolic profile that he had before treatment.
Periodic reassessment matters.
What About Libido and Sexual Function?
Weight, testosterone, cardiovascular health, insulin resistance, sleep quality, mental health, medications, and relationship factors can all affect sexual function.
Emerging research suggests GLP-1 receptor agonists may have favorable effects on testosterone and some measures of male sexual function in men with obesity or diabetes, but the evidence is still developing. Researchers have specifically cautioned that the available literature is not strong enough to prove that GLP-1 medications directly stimulate testicular function.
For a man experiencing low libido or erectile dysfunction, checking testosterone may be appropriate, but testosterone is only one part of a comprehensive evaluation.
GLP-1 vs TRT: They Are Not Competing Treatments
It is tempting to ask whether a man should use a GLP-1 medication or testosterone.
In reality, the better question is: What is causing the problem?
If excess body fat and metabolic dysfunction are suppressing otherwise healthy testosterone production, weight loss may significantly improve hormone levels.
If a man has persistent symptomatic hypogonadism despite addressing reversible causes, testosterone replacement may still be appropriate.
And if both obesity and true testosterone deficiency are present, both conditions can potentially be treated at the same time.
A personalized approach is far more useful than viewing GLP-1 therapy and TRT as interchangeable treatments.
The Bottom Line: GLP-1, Testosterone, and Men’s Health
For men, GLP-1-based therapy can provide benefits that extend beyond a smaller number on the scale.
Research increasingly suggests that significant weight loss may improve testosterone levels in men with obesity-related hormonal suppression. At the same time, weight loss can include some lean mass, making muscle preservation an important part of treatment.
The best results come from looking at the entire picture: body fat, muscle, testosterone, metabolic health, nutrition, exercise, sleep, symptoms, and long-term sustainability.
